An inbound PL/SQL-routed integration needs to send a synchronous response back to the caller, but the response never seems to arrive even though the message is processed.

Answer

Check whether the routing rule has a queue specified — specifying a queue makes the destination call asynchronous (processed later, off a queue) rather than calling the destination address directly and synchronously, so if you need an immediate synchronous response, the routing rule needs no queue configured at all.
