A Crystal Report always prompts for a database login when run through Quick Report, instead of logging in automatically the way other reports do — what's the fix?

Alias the views used in the report's main report and any sub-reports with the correct prefix convention: use \ or _A as a prefix for schema-owner (Application Owner) views, and \\ or _I as a prefix for IAL-owner views. Using the correct alias prefix for each view's actual owner is what enables automatic login for the report, instead of it falling back to prompting the user for credentials.
