The image needs to be placed in a specific subfolder matching the report, not just at the root of report_designer_graphics — the correct structure is MODULE/REP under the report_designer_graphics folder (for example, an image for the INSTANT_INVOICE_REP report in the INVOICE module needs to go in .../report_designer_graphics/INVOICE/INSTANT_INVOICE_REP/, not just dropped at the top level). Getting this MODULE/REP subfolder structure right, inside the folder path defined by the system parameter, is what actually makes the background image or watermark render for that specific report.
