The 'Report Format & Output' popup no longer appears automatically after confirming the Customer Statement of Account screen, even though it used to — how do you get it to display again so the correct layout can be selected?

Answer

Check the relevant customer's Output Media field (found on the Customer screen's Payment tab) — the Report Format & Output popup only appears automatically when Output Media is set to Printout. If it's set to something else (like a non-print delivery method), the popup won't display, which is what makes it look like the report layout selection has gone missing.
