Where are Permission Set descriptions actually stored in the database — querying for permission set names and what's granted to users works, but the descriptions themselves aren't obvious where to find?

They're stored in FND_ROLE — query that table directly (alongside whatever permission-set/grant tables you're already querying for names and grants) to pull the description text as well.
